PRIMHD review data is linked to a referral
It is important to note that SCR collections are linked to a referral � this means that if a person has more than one referral into a qualifying (ie. PRIMHD) team, then a collection must be created for each of those referrals.

PRIMHD review criteria
To create a PRIMHD review, the following conditions must be met:
- Referral status must be Accepted or Recently Discharged,
- You must have access to the team,
- If the team has an end date, it must be in the future.
Once the above criteria have been met, the PRIMHD Reviews widget will appear on the client
dashboard.

Creating a new PRIMHD Review
There are 2 options to create a new PRIMHD review, either through the PRIMHD Reviews widget on the client�s dashboard:
- From the client Dashboard, click Add on the PRIMHD Reviews widget.
or via the Referrals tab
- Select the referral for which you are creating the service review by clicking the View button next to the referral, as shown below.
- Click on the View all PRIMHD reviews link, as shown below, to open the New PRIMHD Service Review window.
- Then click Add.

Entering a PRIMHD Service Review
However, you arrive at it, the New PRIMHD Service Review window always appears as shown below. This is where you will record your SCR data.
Each service review collects these pieces of information:

Review date
This is the date the review occurred. Use the buttons to quick-select either �Today� or �Yesterday�, or select an earlier date using the date-picker.
Note that currently the specification does not explicitly exclude reviews outside the referral dates, so we do not prevent you from recording a review either before or after the referral date for the selected team. If you do so, however, the review may return an error when submitted to PRIMHD.

Next review
Recordbase will remind you when the next review is due. Recordbase currently automatically calculates the next review date for you so you don�t need to think about it. The next review is automatically set for 90 days away but this can be changed for your organisation if you wish.

Which team?
If the person for whom you are performing this review has multiple referrals to eligible teams, then you need to specify the team to which this review relates. The list of teams is filtered to those for whom the person has an active referral. If there is only one open accepted team then this will default in automatically

Service review questions
Then follows a series of questions, answered by clicking on the respective button. Some questions will require further answers to be specified, based on selection. For example, if the service user is marked as being in paid employment, then the system will ask you how many hours per week the person is working:
