The Referrals screen summarises a client's referral history within your organization. It offers a snapshot of the teams to which they have been, or are currently referred, including any hospitalization or absences from a team.
Referral Guidelines
Start dates of new referrals can be backdated, however, referrals cannot be future-dated. The referral cannot be created before the initial referral received date.
Additional Referral Information
Transfer
This referral occurs when a client transfers from one team to another team.
If you are transferring to a team that requires more mandatory information for its referrals than the source team, then you will be prompted to enter that information when you perform the transfer. A common example of this is transferring from a team that does not require an NHI number to one that does. You will be prompted to enter an NHI number for the client being transferred.
Concurrent
This is when a client has open referrals for multiple teams at the same time.
A client cannot have a referral for a Respite and Residential team at the same time, nor can they be in two Residential teams simultaneously.
A client can be in as many community-based teams as required if they are different.
A client can be referred to a Residential or Respite team and have a concurrent referral to Community-based teams if the community-based teams are different.
Absence
This referral records when a client is temporarily away from a team.
Hospitalisation
This referral is used when a client is admitted to the hospital.
