Creating an Outcomes tools record

To create a new outcome tool, from the client screen click Outcomes Tools on the left-hand side menu.

Select the desired Outcome Tool from the dropdown list. This list will show all outcome tools available to the team the client is referred into.

• The number in brackets beside the Outcome Tool is how many instances it has been completed.
• Click New.
• Some Outcome tools are linked to a single referral. If that is the case for the assessment you are creating, Recordbase will prompt you to select the referral to which the assessment relates. There is also the option of having the outcome tool restricted to a team. This is an optional field that needs to be enabled by Wild Bamboo Support. Restricting by team means other teams won't have access to that completed outcome tool.
• Enter the appropriate information. Click Save.
